Partners for Patriots Service Dog Application

The mission of the Partners for Patriots organization is to obtain, train, and provide service dogs to

disabled American Veterans to help them with emotional and physical independence.

ONLINE APPLICATION PROCESS

Complete all information on this online application and submit or download a printable PDF application.

A representative from Partners for Patriots will contact you within 2 weeks after we receive your

application for an interview. Our time frame is a 1 to 3 year wait but we try to provide the dogs as soon

as possible. The trainer will work with the applicant and their dog for a total of 3 to 10 days. If you
